CSB Chides Oil and Gas Well Operator for Death of Worker, Wife

An Odessa, Texas, oil and gas well management company failed to adequately vent a water pump house, allowing a toxic buildup of hydrogen sulfide that killed one of its workers—and then his wife who came looking for him—the U.S. Chemical Safety and Hazard Investigation Board said in announcing the end of its probe of the mishap.

“In my view this is a major oversight on the part of the company and all other companies that have the potential to release toxic chemicals,” said Katherine Lemos, chair of the safety board during a Tuesday call with the public.
